Keynote: Metadichol®: A novel VDR inverse agonist and applications in skin diseases
Biography:
P R Raghavan is the CEO of Nanorx Inc. He has completed his PhD in Organic Chemistry from Oregon State University (1979) and an MS in Chemistry (1972) from IIT Mumbai, India. He has worked on drug discovery for over 25 years at Columbia University, Max-Planck Institute, Germany, Ciba-Geigy (now Novartis) and Boehringer Ingelheim. He has over 15 US and international patents and another 15 pending patent applications.
Abstract:
Metadichol (US patents 8,722,093 and 9,043,383) is a nano emulsion of long-chain alcohols found in many foods that is present in foods such as rice, sugar cane, wheat, peanuts. The skin is unique in being not only the source of vitamin D for the body but also capable of responding to the active metabolite of vitamin D, 1,25(OH)2D. Both 1,25(OH)2D and its receptor (VDR) play essential roles in the skin. unique relationship that entangles vitamin D to dermatology. On one hand, our skin is one source for this important vitamin and on the other hand all available data point to its important impact on the health of our skin and the involvement of its deficiency in the pathway of many dermatological diseases. Metadichol acts as inverse/Protean agonist on Nuclear Vitamin D receptors (VDR) that are present in cells throughout the body to stimulate the immune system. VDR signaling has been shown to be important in the immune system as well as the skin, particularly in keratinocytes, regarding skin Homeostasis and epidermal barrier function. Metadichol also shows cross reactivity against other nuclear receptors and how this leads to mitigation of skin infections including gene expression analysis and human clinical case studies will be presented.

He currently serves as Medical director at Ideal Image Central Florida (Laser Hair Removal Company) and Hair Loss Control Clinic (HLCC). Abstract:
Statement of the Problem: Millions of patients worldwide are seeking treatment for hyperpigmentation. Many of these patients, however are not satisfied with the degree of improvement from the treatment they received. Hyperpigmentaion may result from various underlying causes which can include acne, inflammation,underlying medical conditions amongst others. One of the most common forms of hyprpigmentation encountered by the practicing Dermatologist is Melasma. In order to treat any clinical disorder of hyperpigmentation succesfully, One must understand Melanogenesis or the process of melanin synthesis. By understanding the melanogenesis pathway the clinician will be better equipped to provide optimum therapeutic options to enhance clinical results. Skin whitening may be much better achieved with greater success by comprehending the melanogenesis pathway and therapeutic interventions done at various points in this very complex pathway. The use of the most commonly utilized ingredients in skin whitening will be discussed such as Glutathione, Vitamin C, retinoids, hydoquinone and others.

Abstract:
The way patients choose physicians is changing, so physicians need to change the way they grow their practices. Dermatologists are their own brand and business. Every successful business should have a plan. The most important way to grow your practice is through physician referrals. A couple of ways to do this are: use your EMR tools and social media to connect with referring physicians. Some practices are hiring a physician liaison. Another way to grow is by improving customer service, make sure your patients get the best experience possible. Make it easy for the patient by checking in on mobile, electronic payment, easy ability for patients to review and share their experiences. The most successful practices have their staff involved in the strategy, goals, and action plan. The most successful physicians and practices have their staff support. Grow your practice by adding services that patients are requesting, and that they are currently leaving your practice to receive. Also you could add additional providers, someone with different skill sets. It will help increase the scope of the practice. Some other technology to add is telemedicine, staff scheduling software, and reward programs. The heart of a successful medical practice will never change. Who you are and what you do can be adapted to excel in a changing healthcare environment. Extending relationships and building your personal brand via online media, as well as keeping up with the latest changes in treatment options are ways of providing continued growth. If combine these with excellent relationships with your referral sources, great customer service, and excellent brand recognition will help grow you practice.

Keynote: Trifocal technique for hyaluronic acid injection

Ayman El-Attar is the President of the American Aesthetic Association. He founded Derma Laser Centers of New Jersey established in October 2002. He has Graduated from Alexandria Medical School with honors in 1987. He has completed residencies in both General Surgery and Family Medicine, and obtained a Master’s Degree of Surgery in 1992. He was an Assistant Lecturer of Surgery at Alexandria University, and a Visiting Instructor of Surgery at the Medical College of Ohio. He then finalized his PhD thesis in 1998.
Abstract:
Introduction: The use of dermal fillers containing hyaluronic acid (HA) gel to rejuvenate facial skin has been commonly and successfully used for many years. The science behind HA is simple: it is a naturally occurring substance found in cell and tissue fluids, and is a key component of well-moisturized skin. The skin’s high-water content helps to keep it plump and pliable and therefore, HA fillers are a very effective tool for lip and cheek augmentation and correcting lines, wrinkles and folds
Materials & Methods: Ten patients, nine females and one male aged 32 to 55, with class I to III facial aging, according to Fitzpatrick’s wrinkle classification. After a careful explanation of technique and obtaining an informed consent, patients underwent Liquid facelift using Trifocal technique. Four to 8 ml of high cross-linked hyaluronic acid is injected utilizing 2-inch 25-gauge blunt cannula through three access points. Lateral eyebrow access point is used to fill temporal hollowness and raise eyebrow. Access point between vertical lateral epicanthal line and horizontal columellar line is used to fill tear trough, marionette line, cheek, malar atrophy, nasolabial fold, mandibular angle, upper and lower lip. Tip of the nose access point is used to fill nose tip, dorsum and glabellar creases
Results: Technique was well tolerated by all patients. The results produced were statistically analyzed and resulted in a significant and long-lasting effect on facial rejuvenation. Evaluation of photographs before and after the treatment revealed significant clinical improvement in facial wrinkles and hollowness.
Conclusion: Trifocal technique is a novel, safe and effective method for Liquid facelift. Multisyringe injection of hyaluronic acid filler into the aging face results in significant improvement facial wrinkles and hollowness. Liquid facelift with hyaluronic acid is an important procedure in the armamentarium of anti-aging techniques.
